September is Healthy Aging Month, an opportunity to recognize that healthy aging is about much more than a number. It is about maintaining health, independence, and quality of life while continuing to do the things that bring meaning and connection to everyday life.

Healthy aging is a lifelong process, and it is never too late to make positive changes. Regular physical activity, nutritious food, quality sleep, preventive care and meaningful social connections can all play an important role in supporting health as we grow older.

One of the most important steps in healthy aging is staying connected with your health care team. Regular visits can help identify potential health concerns early, keep preventive care on track, and provide an opportunity to review medications and manage chronic conditions.

For adults age 65 and older, Community Health & Wellness Partners (CHWP) offers senior-focused services designed to support patients throughout the aging process.

CHWP’s Annual Care Visit provides a comprehensive opportunity to look at an individual’s health and identify areas that may need attention. Before the visit, a nurse familiar with senior care contacts the patient to discuss medications, needed screenings, health risks and other health needs. That information is then used to help develop a personalized plan for the year ahead.

The Annual Care Visit includes a comprehensive physical assessment, medication review, preventive and diagnostic testing when appropriate and assessments for cognitive and mental health concerns such as memory loss and depression. The care team can also help connect patients with additional services, including nutrition counseling, physical therapy and chronic care management.

For seniors managing multiple chronic conditions, additional support can make a meaningful difference.

CHWP’s Chronic Care Management program offers support between office visits for patients living with conditions such as diabetes, asthma, high blood pressure, heart failure, COPD or kidney disease.

Participants receive an individualized care plan, nurse check-ins, and help coordinating care with other doctors and specialists. The goal is to help patients better understand and manage their health while supporting them at home and in their daily lives.

At CHWP, that whole-person approach is central to its mission of providing quality, patient-centered health care to anyone and everyone in the community. The organization’s senior care services are designed not simply to treat illness, but to help older adults stay as healthy and independent as possible.
