The Logan County Quick Response Team (QRT) continues working together to ensure no one falls through the cracks when it comes to substance use and recovery support.
The team announces a new, public-facing referral link that makes it easier than ever to connect someone to help.
This tool enables community members, family and friends; employers or coworkers; and self-referrals to request a QRT follow-up for someone who may be struggling with substance use—or if you’re unsure where to start but know support is needed.
Access the referral form here: https://form.jotform.com/242474665625161?external_source_id=OH0460000X or simply scan the QR code in the flyer to get started.
An important tip: When completing the form, scroll all the way down in the “Referring Agency” dropdown and select: “Citizen” (for community/family referrals), or “Self” (for personal referrals).
This referral form is available 24/7/365, does not require a login, and works on any device. Once submitted, the QRT team receives the referral immediately and will follow up as soon as possible, usually in 24-72 hours, to offer support and connection to services.
The Quick Response Team is made up of dedicated partners, including the MHDAS Board, TCN Behavioral Health, Mary Rutan Hospital, Coleman Health Services, Russells Point Police Department, Logan County Sheriff’s Office, Logan County Health District, Recovery Zone and Community Health & Wellness Partners.
“You don’t have to navigate this alone. Whether you’re concerned about a loved one — or yourself — this is a simple way to get connected to help, hope and recovery,” QRT members said.
